In regards to Mr Smith Regarding Mr Smith , a 45-year-old school teacher, I am writing to refer him for further management.
Unfortunately, he has got high sugar levels he has hyperglycemia and requires urgent attention. His BMI is 32 and he admits to a poor diet.
Please monitor him. Your monitoring of his blood glucose would be appreciated.

"In regards to" is slightly informal and wordy. "Regarding" is sharper and more professional for a medical referral.
Use clinical terminology. "High sugar levels" is layman's language; "hyperglycemia" demonstrates professional lexical resource.
Direct commands ("Please monitor") can sound rude to a senior colleague. Use passive voice or polite requests to improve your Genre & Style score.
